http://www.siouxfalls.org/FCHIt is the mission of Falls Community Health to provide an open door to quality health care services by removing barriers, especially financial, that exist for families without access to medical and dental care. Falls Community Health provides a family practice level of health and dental care based on an "ability to pay" sliding fee schedule. Sliding fee discounts are based on the federal poverty guidelines. Services offered include:*Care for acute and chronic illness *Physical exams and preventative health care check-ups *Well-child exams and immunizations *Routine and emergency dental care *HIV early intervention services and case management *Family planning services *Patient education programs and *Prescription medication discounts.
